As July winds down and August begins, appointments for physicals will start to become scarce. To help yourself out and avoid the last minute paperwork rush, here is the link to the 4 forms required for Fall sports participation.

First day of practice is August 12. Paperwork is due August 9 in order to avoid delays in tryouts. Your daughter will not be able to participate without these documents on file.
